For urgent matters our staff will take your details and contact the doctor as soon as possible. The doctor will triage the matter and contact you when possible.

Recalls
We have a recall system for patients needing to make an appointment for test results, annual health checks, pap smears etc. You will be contacted either by phone, email, text message or letter.

Privacy
Our practice upholds the Australian Privacy Act 1988 in relation to the the collection, use and disclosure of your personal information.

Test Results
Due to patient confidentiality and privacy, test results will not be given over the phone. Please make an appointment to discuss your test results with your Doctor.

Our team may communicate with you via electronic means using third party software. Any information collected is kept confidential and used for patient medical care.

Our staff will triage and admit to the service if suitable

Bulk Billing Conditions

The majority of our doctors’ appointments are Bulk Billed if you:

a) Hold a valid Medicare or DVA Card
b) Some Private Health Insurance organisations with GP cover
c) BUPA Member.

Other patients – A fee may be charged according to the level of consultation needed. Please ask our friendly reception staff for fees and details.

Appointment Information

Our team aim to provide you with the highest quality medical care. To help with this please remember to bring the following to your appointment:

  • Medicare or DVA or Pensioner card
  • List of any medications or supplements you are taking
  • Any recent relevant tests or reports you have undertaken in relation to your condition
  • New patients – Please arrive 10 minutes to fill in a new patient and medical history form (or see form below).

If you need to discuss more than one health issue it may be best to book a long consultation. Please enquire at time of booking if you are unsure.

You may either call during clinic opening hours or book your appointment online 24/7.

Walk-In appointments – Are available if a Doctor has time in their appointment schedule. Where possible please call reception before visiting.

Bulk Billing Doctor Ballarat

The majority of our doctors’ medical consultations are Bulk Billed if you hold:

  • A valid Medicare Card or DVA Card
  • Some Private Insurance organisations with GP cover
  • BUPA Private Health Insurance.

Please note – Some types of medicals (e.g. drivers medical) are not covered by Medicare and are charged privately. Please ask at reception if you are unsure.

Non-Medicare Card or Health Insurance Holders

Non-card holders and overseas visitors without private medical insurance – An out of pocket fee may apply according to the level of consultation undertaken. Please contact reception for fees and details.

Dr Mohmad Faahimi

  • MBBS – Manipal University.

Dr Faahimi:

  • Graduated from Manipal university and began his internship in Malaysia in Orthopaedic Surgery for 8 years.
  • He recently moved to Australia and has worked in Frankston Hospital emergency and medical departments.

Dr Faahimi has a special interest in:

  • Orthopaedics
  • General Medicine and;
  • Minor surgical procedures.

Dr Faahimi is fluent in:

  • English and;
  • Malay.

Dr. Swathi Choyi

Dr Swathi holds the following qualifications:

  • Australian Medical Council Australia – 2015
  • MBBS – Bachelor of Medicine / Bachelor of Surgery Amrita Vishwa Vidyapeetham University India – 2014.
  • Bsc. Biomed.

Dr Swathi has:

  • Been practicing in both India and Australia in a hospital setting for 10 years.
  • Over the past 3 years she has been working as a general practitioner in South East Melbourne before coming to Ballarat.

Dr Swathi has a special interest in:
